The waste generator or authorised person who sends waste from his/her establishment or facility shall be obliged to report each shipment of hazardous waste to a single unloading point separately before the shipment commences, within the scope of the reporting form.

The operator of a waste collection or disposal facility who receives equipment containing PCBs above 5 litres of charge shall report the details of that equipment to the Ministry of the Environment.

Operators of facilities carrying out interim storage of mercury waste and operators of facilities carrying out conversion and, where appropriate, solidification of mercury waste shall send the statement pursuant to Article 14(4) of Regulation (EU) 2017/852 of the European Parliament and of the Council in paper or electronic form to the Ministry of the Environment.

The operator of a pharmacy is obliged to submit to the competent regional authority data on the amount of waste pharmaceuticals from households that he/she has transferred to a waste management facility during the past calendar quarter, in writing on a prescribed form within 30 days of the end of that calendar quarter.

The waste producer, the facility operator and the waste dealer are obliged to keep continuous records. A running record shall be kept separately for each type of waste, for each waste facility, for each waste dealer and for each establishment where waste is generated. In the case of off-site generation of waste and treatment of waste in a mobile waste treatment, recovery or disposal facility, a continuous record shall be kept separately for each administrative district of the municipality with extended competence and the capital city of Prague.
