Obligation to disclose information - The service provider is obliged to disclose the following information to the recipient of the service before the conclusion of the contract or before the provision of the service: (a) the name and surname or business name, if applicable; (b) its contact details, which are in particular the postal address, fax number or e-mail address and telephone number; (c) information on whether the service provider is registered in a commercial or other similar register, if so, (d) the registered office of the service provider in the case of a legal person, or the place of business of the service provider in the case of a natural person, (e) if his activities are subject to authorisation, the name and address of the authority which issued the authorisation, (f) the tax identification number, if any, (g) information on whether the service provider is a member of a professional chamber, including the professional designation which such membership confers on him, (h) information on the commercial terms or clauses or contractual provisions relating to choice of law, if these govern the relationship between the service provider and the recipient of the service, (i) information on the scope, the terms, conditions and manner of exercising guarantees not provided for by law, if any, (j) the price of the service, if predetermined by the service provider, (k) a basic description of the service provided, (l) information on liability insurance or other guarantees, including contact details of the persons providing them and information on their material and territorial scope. The service provider shall make that information available by means of (a) information available at the place where the service is provided or where the contract is concluded, (b) documents supplied to the recipient of the service describing the service provided, or (c) remote access, in particular by means of an electronic address.

Obligation of the service provider to disclose, at the request of the recipient of the service, prescribed information, e.g. price estimate, existence of professional rules, information on out-of-court dispute resolution. If the price is not fixed in advance; if the price cannot be estimated, the service provider shall at least communicate the method of calculating the price.
