The provision of social services requires authorisation, which is granted by a decision on registration, which is decided by the regional authority (competent according to the place of permanent or registered residence of a natural person or the registered office of a legal entity). One of the conditions for registration is the submission of a written application for registration. Application for registration - data that must be included, documents that the applicant is obliged to attach (e.g. data on the social services provided, e.g. types of social services provided, plan of financial provision of social services).

The obligation of the applicant for registration (for the purpose of providing social services) to meet and document the conditions prescribed by law (professional competence, integrity, proof that the applicant has no tax arrears and no arrears of insurance premiums and penalties for public health insurance and social security premiums and penalties and contributions to state employment policy, etc.).

The social service provider is obliged to notify the registering authority (the competent regional authority) in writing of changes to the data contained in the application for registration and in the documents submitted with the application. The social service provider is obliged to notify these changes by the fifteenth day of the calendar month following the calendar month in which the changes occurred and to document these changes with the relevant documents.

The social service provider shall submit a written request for a change of registration to the registering authority (the competent regional authority) when the data in the registration decision is changed. The application for a change of registration shall contain the data to which the changes relate, supported by the relevant documents.

A social service provider who is registered in the register is obliged to communicate data (data on capacity, material, technical and personnel support, provision of basic and optional activities and financing of individual social services, and generalised data on applicants, persons to whom the social service is provided and persons with whom a contract for the provision of social services could not be concluded, and data on the number of persons to whom restrictive measures have been applied and on the frequency of their application according to the types of such measures, for the purposes of analytical and conceptual work of the Ministry of Labour and Social Affairs) to the regional authority via an electronic system on a form prescribed by the Ministry of Labour and Social Affairs, by 30 June 2009.
