January of the current year, statistical data for the previous calendar year (number of vacancies for which job placement was requested, number of natural persons placed by them, including the number of job seekers placed on the basis of an agreement with the Labour Office, number of their employees who were temporarily assigned to work for the user, with the number of citizens of the Czech Republic being indicated separately, the number of citizens of the European Union, the number of citizens of other Member States of the European Economic Area and Switzerland and the number of other foreigners by nationality, identification of the user to whom the employees of the employment agency were temporarily assigned, the number of employees by groups of the Classification of Occupation (CZ-ISCO) published by the Czech Statistical Office assigned to each user).

Application by legal/natural persons for a permit to provide employment. Obligation of a natural/legal person to provide prescribed information in the application for an employment agency permit (e.g. place and object of business, identification data of the natural person), obligation to attach prescribed documents (e.g. address of its workplaces that will carry out the agency, certificate of integrity of the foreign natural person). The application for a permit for employment mediation is submitted to the Ministry of Labour and Social Affairs.

Application for the establishment of a job for a person with a disability and the granting of a contribution. The application must be accompanied by written documents - e.g. proof of the employer's account at a financial institution, confirmation that the employer has no tax arrears recorded in the tax records kept by the tax or customs office, does not have any arrears of insurance premiums and penalties for public health insurance or social security premiums and penalties and contributions for state employment policy (if the employer gives written consent and exempts the relevant tax or customs office, the relevant district social security administration or the relevant health insurance company from confidentiality with the Labour Office for this purpose, the Labour Office will arrange this itself).
