The applicant for an investment incentive shall, in its intention to obtain an investment incentive, indicate, in the case of a legal person, the identification data of the persons who are the statutory body or a member of the statutory body of the legal person, indicating the manner in which they may act on its behalf, the identification data of the persons, who hold 20 % or more of the voting rights or a business shareholding of 20 % or more of the share capital or, in the case of a controlled person, the identification of the business name or the name and surname of the controlling person and the State in which its registered office is situated.

The applicant for an investment incentive shall indicate in the intention to obtain an investment incentive the main business activity, the size of the applicant, the contact person authorised to act for the applicant in proceedings under the Investment Incentives Act, selected economic data on the applicant and data on the number of employees.

The applicant for an investment incentive shall provide information in the intention to obtain an investment incentive on the investment, its location, the date of its commencement and completion and the method of its financial, personnel and material security.

The applicant for an investment incentive shall provide data on the impact of the investment project on the environment and energy consumption of the energy management according to the law regulating energy management at the location of the investment project in its intention to receive an investment incentive.

The applicant for an investment incentive shall provide details of the forms of investment incentive requested in the intention to receive an investment incentive.
