A feed mill may be operated after approval of an application to the Central Institute for Agricultural Inspection and Testing. Obligation to indicate on the application for approval of the operation: (a) in the case of natural persons, the name or, where appropriate, the first and last name, date of birth or, where appropriate, the business name, the address of the place of permanent residence or other address for service, usually at the place of residence in the territory of the Czech Republic or at the place of business, the identification number of the person (hereinafter referred to as the 'identification number'), if assigned; in the case of legal persons, the trade name or business name, registered office, or other address for service, or, where applicable, the location of the organisational unit in the territory of the Czech Republic, and the identification number, if any; (b) in the case of natural persons established in a third country, the name and surname, date of birth and place of residence of the person responsible in the Czech Republic; in the case of legal persons established in a third country, the registered office of the organisational unit in the Czech Republic or the name, date of birth and place of residence of the person responsible in the Czech Republic, (c) the contact e-mail address or telephone number, if available to the applicant, (d) the address of the establishment, the type of activity and its specification, (e) the data subject to patent protection and the data classified as trade secrets.

A feed mill may be operated after approval of an application to the Central Institute for Agricultural Inspection and Testing. The following annexes and copies of documents must be supplied as part of the application for approval of a feed business on request: (a) data on the feed, (b) the production process, (c) a technological diagram of the production facility with a description and the layout of the production facility, (d) proof of control or calibration of the scales and weighing systems used, (e) a quality control plan, (f) verification of the homogeneity of feed additives in feed and premixtures, (g) data on feed imported from a third country, (h) a written declaration by the importer that the product comes from an establishment which complies with the requirements under Regulation (EC) No. 183/20053c), (i) proof of patent protection, (j) a copy of the certificate of professional competence of the responsible person as referred to in Article 6(2) or of the decision on recognition of professional qualifications as referred to in Article 6(3), (k) proof of business registration, (l) a declaration by the distributor that the feed complies with Regulation (EC) No 183/2005 of the European Parliament and of the Council3c), and (m) a written declaration that a hazard analysis and critical control point (HACCP) has been prepared in accordance with the requirements of Regulation (EC) No 183/2005 of the European Parliament and of the Council/20053

The operator of a feed business shall notify changes to the information given in the application for approval of the business on a form issued by the Central Institute for Agricultural Inspection and Testing within 30 days of the change.

The operator of a feed processing plant is obliged to apply to the Central Institute for Inspection and Testing of Agriculture for registration of the plant. The obligation to state in the application for registration (a) for natural persons, the name, or first and last name, date of birth, or business name, address of permanent residence, or other address intended for service, usually at the place of residence in the territory of the Czech Republic or at the place of business, identification number, if assigned; in the case of legal persons, the trade name or business name, registered office, or other address for service, or, where applicable, the location of the organisational unit in the territory of the Czech Republic, and the identification number, if any, assigned; (b) in the case of natural persons established in a third country, the name and surname, date of birth and place of residence of the person responsible in the Czech Republic; in the case of legal persons established in a third country, the registered office of the organisational unit in the Czech Republic or the name, date of birth and place of residence of the person responsible in the Czech Republic, (c) the address of the establishment, the type of activity and its specification, (d) the data subject to the protection of patent rights and the data designated as trade secrets.

Obligation to provide the following attachments and copies of documents as part of the application for registration of a feed business on request: (a) details of the feed, (b) the manufacturing process, (c) a technological diagram of the production facility with a description and the layout of the production plant, (d) evidence of the control or calibration of the scales and weighing systems used, (e) a quality control plan, (f) verification of the homogeneity of feed additives in feed and premixtures, (g) details of feed imported from a third country, (h) a written declaration by the importer that the product comes from an establishment which complies with the requirements under Regulation (EC) No. 183/20053c), (i) a written declaration that a hazard analysis and critical control point (HACCP) analysis has been carried out or that good practice has been applied in accordance with directly applicable European Union legislation governing feed hygiene requirements3c), (j) proof of patent protection, (k) proof of business registration.
